New system for all requests - Please have a go!
You can now make a request for an appointment, a home visit, a sick note or prescription, send us your BP readings, and much more. You can do this from 7am each work day morning. This means you do not have to queue up at the door or on the telephone. If you dont have access to the internet then we can complete the online form for you.
Please scroll to the bottom of each page, you will first be asked to verify this is not an emergency, you can then log in using your NHS App login or use it without, it is better if you use the NHS app as this will prefill the form and remember your details.
We dont make appointments before a clinician has reveiwed the request. Our appointments are then booked based on medical need and prioritised by clinical staff. We hope this will make booking an appointment easier and more equitable for all.
You can submit an administrative or medical query using your phone, tablet or computer. This will save you time queuing on the telephone and we hope will be a more efficient way of working. A member of the team will respond to you to help you with your request via text, email or phone call.
We will aim to respond within 24 hours to offer you an appointment date and time or advice. If the request is in the future (more than 2 weeks) we may not be in touch straight away.
If the service has reached capacity for safe working you will not be able to send medical request during that time.
Our reception staff can complete this for patients without access to the internet, but please be aware their calls will take longer and therefore it is far quicker for you to complete this yourself if you can, or a member of your family or a carer can submit that on your behalf.
